When pregnancy is not happening

Fertility difficulties can involve either partner, both partners, or an unexplained cause. They are not a personal failure. Assessment should consider the whole situation.

A guide to understand and prepare

When to seek care

Seek advice after a year of trying, or earlier with increasing age, irregular periods, known reproductive conditions or previous cancer treatment. Local referral criteria vary.

Urgent-help guide
Understand the concern

A previous pregnancy does not rule out a later fertility problem. An app estimate of ovulation cannot confirm whether ovulation or conception is occurring.

What assessment may involve

Tests may assess ovulation, the womb and tubes, and semen where relevant. Not everyone needs every test; hormone-test timing depends on the question being asked.

Care options to discuss

Agree the next investigation, how results will be explained and when treatment options will be discussed. Ask about emotional support and local costs before committing to a programme.

These are discussion points, not a prescription. Suitability and availability vary.

A question you may have

Is fertility assessment only for the woman?

No. Assessment should include the relevant reproductive factors of both partners when applicable.

Prepare for your appointment

What to record

Bring cycle dates, time trying, previous pregnancies, operations, medicines and any previous test results.

Questions worth asking

  • Do I need assessment now rather than waiting?

  • Which tests should each of us have, and why?
